For whatever reason, the style does not persist, and the checkbox is not a valid GlyphType, so I can't just override it after the fact. There doesn't seem to be any way to do this other than reversing the order of copying, ie copying the template elements into the original document.

Create output document from source doc instead of template this preserves the formatting of the source document, which was being modified in copying, at the cost of a little bit more complexity fixes #1
